Current Emergency Information

You will be asked to complete a Student Emergency Information Form (August and January) listing all telephone numbers and persons other than parents who may be contacted in an emergency. Each semester you will be asked to do so again. It is very critical that you supply this information and keep the office informed of any such changes. The office must be able to contact parents/guardians if students become ill or have had an accident; therefore, we must know of any change of home address, work status or any change in telephone numbers.
